All analyses were performed using R statistical package, version 4.2.0, for macOS ( ), and statistical significance was set at P < 0.05.Īs shown in Supplementary Table 1, among the 2,353 eligible patients, 1,441 (60.0%) and 942 (40.0%) were waitlisted for LT before (2005.06–2016.05) and after MELD scoring was implemented (2016.06–2021.12). The results from the formula was divided by the number of total 90-day mortality was regarded as reducible wait-list mortality by MELD3.0. First, the possible increased number of LT was calculated with following formula: ∑, the sum of multiplying the difference of DDLT likelihood between different MELD3.0 strata, within the same MELD strata (P B – P A), and the number of deceased patients up-categorized by MELD3.0 (N B-A). 2 using a sub-cohort of patients who were wait-listed during post-MELD era and did not received LDLT within 90 days. To estimate the possible impact of MELD3.0 on the waitlist, we calculated reducible waitlist mortality by the method previously suggested by Kim et al. The mean gain of points by using MELD3.0 was compared with the Student’s t-test. 13 In addition, predictive powers for 90-day mortality of MELD and MELD3.0 were evaluated with calibration plots, which were constructed by smoothing curves from the observed and predicted risk of deaths per decile. Model performance for mortality was compared between MELD and MELD3.0 by calculating Harrel’s C index with a test of statistical difference. Mortality during the 90 days after waitlisting was evaluated using a Kaplan–Meier curve with the log-rank test, stratified by the MELD and MELD3.0 groups. 12 Finally, 2,353 eligible patients were included in the analysis ( Supplementary Fig. Although patients with hepatocellular carcinoma (HCC) fulfilling the Milan criteria received additional MELD points (+4 for MELD ≤ 13, +5 for MELD 14–20, and being assigned 25 points for MELD 21–25) after the MELD era, we did not exclude these patients because additional MELD points might not considerably affect the rate of DDLT in Korea, where patients with MELD < 25 had a very low possibility of DDLT. Exclusion criteria were as follows: 1) age < 18 years at the first waitlisting, 2) prior organ transplantation history or waitlisting for LT, 3) listed for multiorgan transplantation other than liver and kidney, and 4) missing data. The patient list, demographics, and date of death or LT were retrieved from Korean Network for Organ Sharing (KONOS) and merged with institutional data on laboratory results that were necessary for the calculation of MELD and MELD3.0. We performed a retrospective observational study with data from patients who were registered for the approval of LT between June 2005 and December 2021 at Severance Hospital, Korea. 11 This study thus attempted to validate the applicability of MELD3.0 to reduce LT waitlist mortality in Korea. 10 To overcome this organ shortage, living donor liver transplantation (LDLT) is predominant in Korea, with 74.4% of total liver transplantations performed with living donors in 2020. 9 As of 2020, 61% of Korean DDLTs have been performed in patients with an emergency status of 1 or MELD ≥ 38. 8 However, Korea suffers from a more severe organ shortage than Western countries, with 7.62 brain-dead liver donations per 1 million people per year. Korea applied the MELD allocation system in June 2016, which resulted in reduced waitlist mortality from 57.4 to 52.7 deaths per 100 person-years. They reviewed its applicability, but no study has assessed its accuracy across the diversity of liver transplant (LT) circumstances in various countries. 6 developed MELD3.0, which reflects the relationship between sex and the mortality of low albumin due to ascites. 5 Moreover, there is a need to consider liver failure complications such as ascites and encephalopathy, which are difficult to objectify. ![]() 4 In addition, disparities in liver allocation between sexes has been reported in several countries due to overestimated kidney function in women. 2, 3 However, the accuracy of the MELD score for predicting waitlist mortality has been questioned due to recent changes in the characteristics and improvement in treatment of end-stage liver disease. In 2016, UNOS reapplied the score, which added Na to the existing MELD as a liver allocation policy, thereby reducing waiting mortality and increasing the equity of organ use. 1 MELD has been used worldwide since it was adopted by United Network for Organ Sharing (UNOS) as an organ allocation policy in 2002. The model for end-stage liver disease (MELD) is a tool developed for organ allocation in deceased donor liver transplantation (DDLT) based on predicted mortality. ![]() "She can pick up a book and she'll tell you what's happening on the page, sometimes she can even recognize the word. Jordin, who has not started preschool, still has two years before she takes the admissions test, but already her mother has noticed growth. She was teaching the child how to classify objects or determine the relationship between things. She then asked the toddler to pick out whether a peach or a green pepper would fit into a group of plastic fruits. She was teaching visual sequencing, the ability to reason and solve problems using pictures, diagrams and geometric shapes. Tutor Lemi Erinkitola started a tutoring company for kids as young as 3, preparing children, mostly on the South Side, for CPS' admissions tests. On our newsroom blog Trib Nation, how a conversation with parents became a sidebar on how to prepare kids for these tests. This year she hired a former Montessori teacher to tutor her preschooler on everything from learning continents to sounding out words. Even though with no special preparation her daughter made it into Decatur Classical Elementary, a top-scoring school, "we realized we couldn't do that for our second child and just hope to be lucky," she said. "I was blissfully naive about how this all worked when my older daughter tested for first grade," said Shannan Bunting. As kindergarten is an entry year for most of those programs, many parents are hiring private tutors, researching tests used in other large urban school systems, finding age-appropriate questions online and doing whatever else it takes to get their kids on the right track early. We're not letting kids be kids, and we're making them into little Einsteins." But with low-performing neighborhood schools an unattractive option and the cost of some private schools out of reach, many parents see CPS' selective enrollment programs as the best public education option in the city. "This sort of insanity testing produces test coaching for little kids and gaming of the system by parents and others to figure out what's on the test and get their kid a leg up. "It's just yet another example that the country has gone test crazy," said Robert Schaeffer, public education director for FairTest, a national nonprofit that advocates for other methods of assessing young children. You can probably predict what comes next yourself: With 3,337 applications filed for about 500 seats in Chicago Public Schools' classical and gifted kindergarten programs next fall, parents are helping their preschoolers cram for the tests. They could be asked to identify trapezoids, figure out how many cookies they'd have if Mom put two more on their plate, demonstrate advanced literacy skills and, for gifted programs, be able to infer relationships, recognize patterns and predict what comes next. To test into some of Chicago's top schools, incoming kindergartners must be able to do more than just count to 10 or rattle off the alphabet. ![]() Russia seized Crimea from Ukraine in 2014 and claims it as its territory. The couple opened it because their daughter Patti was a competitive skater and she needed a place to practice. Nell was actually approached by the Mayor of Plano and convinced to open a second Thunderbird Roller Rink in Plano in 1972. At that time, there was nothing in Plano for entertainment. Neighborhoods were just starting to be built this far north of Dallas and streets like Spring Creek were still being planned. If this change could happen, I would gladly upgrade. It adds time and complexity that I think could be averted with a smart refinement of the clipping plane function. In this case, the lineweight for the detail section view might require a thicker lineweight, and so Im forced to literally draw over top of the view with a thicker line layer, or make2d and then changespace those elements to bring them into layout view to change the lineweights. I often need to generate overall sections as well as zoomed in details of those same sections to allow for dimensioning and labelling. Customize lineweights of the clipping planes. For example: in a plan section generated from a clipping plane, I could hatch the walls with a 45 degree cross hatch at my specified scale for that hatch (maybe I want the lines tighter together) and the columns with a solid hatch set to a light grey color. The option to hatch clipping plane elements uniquely. So in this ideal version, the clipping plane would: It’s really sad and discourages me from modelling in 3d on many projects. Linetypes are defined by a comma-separated list of dash and gap lengths (in model units), always starting with a dash. Then save this file as your laser template. Place it on a Table layer and lock the layer as well. Set the print width of this rectangle to No Print and lock it. I really dislike having to create make2d and then customizing the line thickness and adding hatching… when I later have to do changes, due to time constraints, I usually just change the 2d linework, leaving the 3d model at this point obselete. Create a laser template in Rhino by opening a blank file and drawing a rectangle the exact size of the cutting table with the origin on the upper left corner. In the example, I put the lines to laser cut on the black layer, so that would be the only layer to export to the laser cutting machine. If I had to pick just one upgrade for Rhino, it would be to make clipping planes much more effective for generating 2d section views in layout. If that’s not the case, you could go back and trim off anything that extends beyond the outline (the inelegant part, ha!). With laser-cutting cardboard, I didn’t think this was crucial or a deal-breaker. The sloppy part about this is that some parts of the 1 mm lines extend beyond the outline shape. Linetypes are defined by a comma-separated list of dash and gap lengths (in model units), always starting with a dash. Linetypes affect only curve objects - not surfaces, solids, or other object types. Using linetypes displays curves using dashed or dotted. Repeat with the 2-segment polyline for the interior folds. The Linetype properties manage the linetype patterns for the current model. ![]() ![]() This distributes the whole number of 1 mm lines evenly along the shape of the star, with a slight variance in the space between the 1 mm lines. I then change to Number of Objects, and type in the value that Distance Between Objects generated (the tool doesn’t actually update the Number of Objects value with the whole number calculated from the Distance Between Objects option, so that’s why I re-enter the same value that’s already there), and hit Enter. When you enter a value in Distance Between Objects, the Number of Objects updates in the background. I first choose Distance Between Objects and plug in 2 mm. The workflow seems overly complex because I have to print the design from Rhino as a PDF. If I export the dashed lines they show up as continuous lines, so I’ve found it necessary to print the dashed lines first as a PDF. I’m creating dashed lines in Rhino that I transfer to the lasercutter software. ![]() I assumed I wanted the laser to cut a 1 mm line, then a 1 mm gap, then cut the next 1 mm line, etc. I’m using Rhino in conjunction with a laser cutter, to make folded cardboard forms. It gives the option to choose the number of objects to array or the distance between objects. Not sure if this is necessary, but I like to use it with Array Along Curve. I used Array Along Curve to distribute the 1 mm line around the star. Using Dir to find start point and direction, I drew a 1 mm line from the start, with a Near snap to keep it on the star outline. I drew a star from the polygon tool and a two-segment polyline from the tip of a ray, to the origin, to inside corner between rays. Not sure what you’re making, but I imagined a 5-point star. Admittedly a little inelegant or sloppy, it might fit the bill. Based on your comments, it’s an all-Rhino solution that would be digestible for students. I was intrigued by your topic and wanted to try out an idea. ![]() These examples are programmatically compiled from various online sources to illustrate current usage of the word 'sigma.'
